Benefits Summary
Health Insurance:
MBF offers two health insurance plans to all active/full time employees. We offer a base plan and a buy up plan, both of which have nationwide coverage. Insurance is effective on the 1st of the month following hire date. MBF pays a major portion of the cost for employee coverage. Dependent coverage is available at an additional cost.
Dental Insurance:
MBF offers two dental insurance plans to all active/full time employees. We offer a low plan (basic coverage) and a high plan (enhanced coverage). Insurance is effective on the 1st of the month following hire date. At this time, MBF pays a portion of the cost for employee coverage. Spousal and dependent coverage is available at the employee’s expense.
Vision Insurance:
MBF offers low cost vision insurance to all active/full time employees. Insurance is effective on the 1st of the month following hire date. Vision insurance is paid for by the employee. Spousal and dependent coverage is also available.
Life Insurance:
Supplemental Life Insurance is offered at a group rate for all active/full time employees and their family members. Coverage can be purchased in the amounts of $10,000 to $500,000 at the employee’s expense. Any coverage in excess of $140,000 will require additional information and/or a physical for approval.
401(k) Retirement Plan:
MBF offers a 401(k) retirement plan to all active/full time employees over the age of 21. Participants can start making contribution to the plan immediately. After 1 year of service, participants are eligible for company matching contribution. At this time, MBF matches dollar for dollar on the first 3% and 50 cents on the dollar on the next 2%. Thus, if participant contribute 5% or more, MBF will contribute 4%.
